REGISTRATION FORM

To book a performance, please submit your registration form no later than THREE WEEKS prior to your chosen time. Performers are welcome to join on any of the booked dates as long as time allows. You will recieve a confirmation email from us once you submit the registration form. Click here to see the program of each performance.

Please note that the booked concerts are all on SUNDAY. They start at 3:00 PM

***Please send us your program NO LATER than TWO WEEKS PRIOR to your performance day. Melody Express reserves the right to alter or cancel performers if no program is recieved in time.***
